The pharmacy doesn’t manage risks effectively. It doesn't have up-to-date procedures which tell its staff how to work safely.

All SOPS will be reviewed and, updated to reflect current practice. All SOPs will then be read and signed by the relevant staff members.

The way dispensing errors are recorded will be reviewed to improve the level of detail recorded. This will assist staff to think more about how and why a mistake happened and what improvements should be made.

The pharmacy doesn’t make all the records it needs to by law within the required timescales. And it doesn’t have adequate processes in place to investigate any discrepancies in its records promptly.

The frequency of controlled drugs will be increased to enable the pharmacy to detect any discrepancies early. The importance of which will be made clear to any locums also. Date checks will also be carried out more frequently to detect any expired stock. These will be clearly separated to minimise dispensing errors.

All private prescriptions will be entered either the same day or next day. Entries will always include all the required information including the date on the prescription and prescriber details.

The minimum and maximum temperatures of both fridges used for storing medicines will be checked and recorded on a daily basis.

The pharmacy doesn't have robust systems for disposing of all confidential waste effectively. And the way that it stores some prescriptions increases the chances that people’s information could be disclosed in error.

The pharmacy will sign up to a confidential waste disposal service, ensuring that any confidential waste is thoroughly destroyed in the correct manner. All staff including locums will be informed of the importance of any new processes, and the importance of GDPR. Additional training on GDPR will also be provided to all staff.

The storage arrangements for prescriptions will be reviewed to minimise the risk of data breaches.

The pharmacy cannot show that all medicines which require refrigeration are stored at the correct temperatures.

The minimum and maximum temperatures will be checked and recorded on a daily basis for both fridges used to store medicines. An appropriate thermometer will be obtained if needed.
