The pharmacy is not identifying and managing risks associated with the services it provides.

Arrange conference call with key stakeholders to discuss inspection report and actions required.
Arrange an evening meeting with the pharmacy team to review the content of the inspection report and the plan to improve and maintain standards.
Meeting agenda to include:
•What a “standards not met” inspection means; plan to improve standards and the responsibility to maintain.
•Near miss recording, review and learning, utilising Datix.
•PSI review, learning and improvement actions following incidents.
•Staffing skill mix and rota, field support process.
•Raising concerns process.
•The importance of tidy workspace and model day. How this reduces the risk.

The pharmacy does not have enough team members to operate its services safely and effectively.

Review of staffing levels and skill mix (using staffing and capability assessment document) Completion of staffing rota, matrix and training plan.
Complete and maintain 6 weeks forward planning rotas
Assess against workload and identify any shortfalls. Escalate to RDM.
Recruitment completed for vacancies. Full induction to be completed for new starters upon commencement of employment.
Pharmacist recruited to work full time in the pharmacy to provide continuity.
Locum dispenser to continue employment until induction of new starters completed.

There is insufficient evidence that appropriate action has been taken to address the concerns raised by members of the pharmacy team.

Review Regional workload planning and how the team escalate concerns and determine any improvements.
RDM to maintain contact with the pharmacy on a weekly basis ‘How you doing – check in’
Team to be briefed on raising concerns and whistleblowing process.
Discuss any outstanding concerns raised by the pharmacy team during team meeting.

The dispensary is untidy and cluttered. And this may increase the risk of dispensing errors or accidents.

Discuss workflow, One Standard and Best in Class Prescription Management and Stock Management in the team meeting to address benches cluttered, baskets stacked etc.
Complete out of hours work to address backlog of workload and tidy the store.Complete full clean of pharmacy during out of hours work.
Ensure cleaning rota in place.

Pharmacy medicines are not stored tidily or in an organised fashion and this may increase the risk of dispensing errors.

Complete full date check of stock, tidy and reorganise the pharmacy during out of hours working.
Implement stock management process to ensure stock levels maintained to required level and reduce current stock holding.
