1.1	The pharmacy does not identify and manage risk appropriately. The pharmacy team do not routinely record near miss mistakes.	We have been asking everyone to fill every single near miss now. Every single member of the team will have their own individual near miss sheets. They will all fill it up promptly and near miss sheets will be reviewed at the end of every month to review the common errors that’s happening repeatedly and what can be done to avoid them. All the staff has been made aware of the importance of this process in a team meeting.	23/01/2023	05/01/2023
4.4	The pharmacy team are unable to demonstrate that concerns are raised when medicines or medical devices are not fit for purpose.	We have also created a new drug alert folder and staff has been reiterated the importance of drug alerts and the potential to cause the harm if drug alerts are not acted upon.	23/01/2023	05/01/2023
