The team members respond to this feedback. And they use it to improve the efficient delivery of pharmacy services. The team members have training and guidance to respond to safeguarding concerns. They discuss the errors and they take the action needed to help prevent similar errors happening again. But they don’t always record their errors. So, the team may miss opportunities to help identify patterns and reduce mistakes.
