The pharmacy's procedure for its cryotherapy service does not consider the training provider's requirement for GP approval for mole removal. The pharmacy's risk assessment for the service also does not take into consideration high risk lesions and how the pharmacy would identify and manage these.

SOP SSOP‑012 updated to include:
- GP referral pathway
- High‑risk lesion identification
- STOP rules
- Structured assessment (Fitzpatrick, lesion description, duration, symptoms)
- Map My Mole specialist referral
Risk assessment updated. New consent form, GP referral, red‑flag refusal, specialist review and follow‑up forms implemented. Staff retrained.
