For the supply of P medicines, we have implemented some changes to show the approval of each P item that is supplied. This is reflected in the changes we have made to the SOP for supply of P and GSL in section 13. This change should ensure safe and effective supply of P medicines alongside record keeping. With the addition of this section there are clear instructions for the RP and pharmacy team on the supply of medicines. An overall risk assessment has been completed. A risk assessment and SOP has been done for the supply of salbutamol inhalers. In the SOP for prescribing in weight loss, a section has been added with instructions and guidance on how to record prescribing decisions. |

To show clear and concise records for products supplied in regard to weight management we have added a section to the SOP. This section mentions the need for and importance of record making in the safe provision of medication. The pharmacy team have had a meeting to discuss how to improve the record making part of the consultation. We have gone through our learnings from the Independent prescribing course and the weight loss training we attended. Furthermore, we have sought guidance from fellow prescribing practitioners to improve in this area. When making a prescribing decision, the reason will be stated for each consultation which will be kept on the patient record. With regards to the P medicines, as mentioned previously for standard 1.1, the responsible pharmacist will go through each order as they normally do, but they will routinely add a note regarding the approval of P medicines, especially the ones that can be misused. |
