The patient screening process is to include a ‘checklist initial assessment’ to verify if the patient does qualify for the service. This checklist is to be stored securely at the pharmacy premesis for audit purposes and good record keeping practice. Details of any exclusions from the service are to be documented and any referals made to other healthcare providers for the patient to consider also recorded. |
