The pharmacy does not fully manage the risks associated with its supplies of higher-risk medicines. It sometimes supplies weight-management medicines to people under a patient group direction (PGD) after an initial virtual consultation. But the PGD requires the person to undergo an initial assessment face-to-face with the pharmacist. The pharmacy's risk assessment (RA) does not fully reflect the nature of the service including that medicines are supplied against a PGD rather than prescribed to people.

With immediate effect. All initial supplies of weight management medicines are to be conducted with a consultation face to face.

No supplies going forward are to be made without the necessary initial face to face consultation to allow a fully informed decision to be made on eligibility and correct verification of the patient.

The Risk assessment will be updated to reflect the changes required and detail the service being provided is under a PGD.

The pharmacy does not keep all the records it needs to for the safe provision of its weight management service. It does not make conteporaneous consultation records to show that it has considered inclusion or exclusion criteria or safety netting advice provided to people taking weight management medicines.

- That valid informed consent was given
- Name of individual, address, date of birth and GP
- Name of healthcare practitioner
name and brand of medication
- Date of supply, dose, form and route
quantity supplied
- Batch number and expiry date
- If applicable anatomical site of injection
- Advice given, including advice given if excluded or declines treatment
- Details of any adverse drug reactions and actions taken
- Supplied via PGD

Records kept:
Adults aged 18 years and over keep records for 8 years

Children aged under 18 years keep records until the 25th
birthday (if the patient was 17 years when treatment was
finished, keep records until the 26th birthday)

The pharmacy cannot provide assurances that its services are delivered safely and effectively. It is unable to demonstrate that it has reliable systems to capture all the relevant information needed for the healthcare professional to make an informed decision whether its safe to provide such medicines to people.

The patient screening process is to include a ‘checklist initial assessment’ to verify if the patient does qualify for the service. This checklist is to be stored securely at the pharmacy premesis for audit purposes and good record keeping practice.

Details of any exclusions from the service are to be documented and any referals made to other healthcare providers for the patient to consider also recorded.
