The pharmacy doesn’t have a set of standard operating procedures which always reflect the way team members work. For example, there has been no update to the dispensing procedure following the implementation of a patient medication record system which alters the way the team dispenses. And the team do not always follow key aspects of the pharmacy’s procedures. This includes for near miss recording and delivery of medicines.

- Review and update the standard operating procedures (SOPs) - specifically update the dispensing procedure so it relates to the PMR system.
- A new SOP for Auditing of staff adherence to SOPs will be implemented.

The pharmacy does not have adequately robust processes to record, monitor and learn from errors made whilst dispensing. This means team members may miss opportunities to identify repeated mistakes and make services safer.

- Take all opportunities to use the near miss record for any error or mistake to help identify repeated mistakes.
- Hold regular meeting to review and discuss recorded errors, including considering the root causes.
- Ensure each member of the team to work under their own account in PMR and applies initials to work such as compliance packs to maintain an audit trail of all dispensing.
