The pharmacy keeps records of its consultations with people but these records do not contain enough detailed information to make it clear why a prescribing decision was made.

- Communication to and training provided (02 May and 06 May) to all prescribers to ensure that adequate records are made following consultations and clear documentation of clinical rationale.
-Level of documentation required for consultation and clinical considerations to be included in the induction workshop to demonstrate the level of detail required
-New prescribers pro-actively audited in their first month to ensure adequate documentation.
-Regular internal audits to ensure meeting the documentation requirements with feedback.

The pharmacy does not always independently verify information about people's body mass index (BMI). And it does not always complete reviews with people as per its prescribing policies. This means some people may be receiving treatment which is inappropriate for them.

-All new patients will receive a prescriber video consultation prior to the approval of any medication
-Patient weight and height will be verified during this live video consultation and ID will be checked.
-‘Active’ patient verification (patients who have ordered in the last two months) is being monitored weekly by Head of Pharmacy Operations in line with prescribing policy.
