Amiry & Gilbride Pharmacy & Travel Clinic (9012263) - Improvement action plan

Standard not met Reason Action being taken by the Pharmacy By when Notification By Pharmacy Improvements Made
1.2

The pharmacy does not have appropriate monitoring arrangements for all its dispensing procedures to ensure its team members adequately learn from their mistakes and make improvements to keep services safe.

The staff have access to Reportsmart as part of the Pharmsmart computer programme and this allows the recording and reviewing of errors. These records are reviewed on a monthly basis by the respective managers, the SI and the Area Manager and teams have meetings to discuss errors and implement improvements to keep services safe.

1.3

The pharmacy does not have an operating structure that clearly identifies where responsibility lies for the services it provides.

In order to meet section 10 the plan is as follows:
The building is in two floors and both have secure separate entrances to each floor. The proposal is to de-register the upstairs from the premises relating to Amiry & Gilbride.
To submit a new premises registration of the upstairs to Gilbride Pharmacy and there each company will dispense for itself.
The operations will be mirror images in terms of processes upstairs and downstairs however with 2 separate responsible pharmacists but following the same standard operating procedures. As discussed we will need to install new dispensary, upgrade technology as such.
To clarify, the structure of the hub is as follows: it has P2P operations managed by Karen Whiteside, and PillPouch operation is managed by Dawn Parry and both have experience and knowledge of the fields they operate in.
The Area Development Manager (Duncan Ross) is kept up to date with what is happening and any issues that are raised are flagged to the Superintendent (Mahyar Nickkho-Amiry) for Amiry & Gilbride.
The hub operation and its accountability goes to Duncan Ross (Area Development Manager). Both pharmacy groups are owned by the same parent company. Whilst they have two separate superintendents, Mahyar Nickkho-Amiry is the head Superintendent and both pharmacy groups operate under the same SOPs and procedures

1.1

Team members are referring to SOPs that are out of date and some SOPs have not been approved by the SI. So there is a risk team members may not be following appropriate procedures.

SOPs shall be reviewed such that they are in date and approved by the SI.

4.2

The pharmacy does not have adequate accountability structures in place to show that pharmacy services are provided safely and effectively.

As a hub it does not provide any pharmacy services. It is a centralised dispensing hub with SOPs relating to these activities.
