The pharmacy does not have a robust process to monitor or audit the safety and quality of its prescribing service, for example by doing regular clinical audits. The pharmacy does not monitor whether its own procedures are being followed, and there is evidence that they are not always complied with.

Conduct regular audits and reviews of prescribing practices, patient outcomes, and adherence to policies and procedures.
Use feedback from audits, patient satisfaction surveys, and adverse event reports to drive continuous improvement initiatives.

The pharmacy does not adequately identify and manage the risks associated with its prescribing service. Its risk assessment does not consider the individual medicines which are prescribed, and it does not have prescribing policies for these medicines. The pharmacy cannot always sufficiently demonstrate that it relies on robust clinical evidence relevant to the UK when prescribing unlicensed medicines.

Risk Identification:
Conduct a thorough risk assessment of the HRT prescribing service, considering potential risks such as, adverse reactions, GP communication, record keeping, adherence to SOPs, risk of each drug prescribed, training, existence of protocols, and policies.
Policy and Procedure Review:

Write policies, procedures, and guidelines related to LDN, thyroid, weight control medication, prescribing, and management.
Ensure policies align with current evidence-based practice guidelines, regulatory requirements, and best management practices.
Identify areas for improvement and update policies as needed to mitigate risks.

Internal training about thyroid management and guidelines as well as weight management, thyroid treatments, and LDN.

Patient Assessment and Monitoring:
Develop standardised protocols for patient assessment and monitoring before and during treatment, including:
Comprehensive medical history and documented external diagnosis
Baseline investigations (e.g., lipid profile, blood pressure, BMI) for weight management
Implement systems to ensure timely and accurate documentation of patient assessments and monitoring results.
