1.6 | The pharmacy does not always keep consultation records for medicines prescribed by the pharmacist. This includes documentation of the consultation with the person, examination records, and any follow up or monitoring that needs to be done. |
As a company we have had a briefing/meeting with all Pharmacist Independent Prescribers delivering a Pharmacy First Plus Service to share ideas on how we can best keep consultation records. The Pharmacy has taken on feedback and is aware of how important consultation records are in order to record examination records, results and any follow up or monitoring needed in order to deliver the highest standard of care and patient safety. The Pharmacy is currently in the process of putting provisions in place to free up time for the Pharmacist to complete these consultations accurately. A member of the Dispensing Team is undertaking her ACDA Accuracy Checking Course, allowing the Pharmacist to focus on delivering this service. The Pharmacist has completed a CPD Cycle on the importance of up to date and accurate record keeping and recognises how vital this is in the continuity of care for the patient. |
