The pharmacy team will conduct a full review of the risk assessments. Risk assessments will be completed for each group of medication including weight loss medication and off label prescribing. These risk assessments will be reviewed every 6 months and immediately if there is an incident. |

In person (face to face) consultations with the patients will be audited by verifying with the patients. For weight loss medication we now require BMI for each patient which will be assessed with their next prescription to ensure supply is suitable. This will be recorded on the PMR. Checks will be made to ensure the prescribers are following the correct protocol and dosing schedule for weightloss medication. Any ‘off label’ prescribing will be queried with the prescriber to ensure there is no suitable licensed alternative. When a prescription is for a nominated practitioner the prescriber will be asked to provide a patient specific direction on the prescription. All clinical interventions and their outcome recorded and audited monthly to show prescribing trends. |
