The pharmacy does not have sufficiently robust procedures for managing its medicines in the way it should. And it does not always store them in the appropriate packaging.

. The Pharmacy has added advisory warnings to compliance pack labelling. And Patient medication Record (PMR) settings updated.

2. All compliance packs will be supplied at the beginning of each cycle with leaflets to ensure that patients have additional information about their medicine and to help them take their medicines properly. This has now been implemented.

3. The pharmacy will review its procedures for storing split packs, and these will be kept in original containers or marked up accordingly. Staff have been retrained in the correct procedures to follow. This is to ensure that different batches of tablets in stock will not be mixed together. Additional checks will be made to prevent any immediate risks until the improvements have been embedded.

4. The pharmacy has carried out a full expiry date check of all stock. And a new date-checking rota has been set up to ensure regular monthly date checks. The process will be audited by a senior manager from 5th April.

5. All staff have been re-trained on the correct procedures for reading, recording and monitoring fridge temperatures.

6. Procedures for the management of CDs have been reviewed.
