Pharmacy team members are not following the systems in place to learn from mistakes. And, they do not make changes to help stop mistakes happening again.

A Royal Pharmaceutical Society Near Miss Log will be used and staff encouraged to document ALL Near Misses in the dispensary. The Responsible Pharmacist will ensure staff complete this record themselves. This form will prompt staff to record possible causes of the error and record action taken immediately after it has occurred.
The Team will huddle and use the Near Miss improvement tool provided by the RPS to reflect on errors made, why they were made, record any actions taken and review actions taken to prevent them from happening again. This will be recorded on the form. The frequency of reviews will be monthly; or more frequently if needed.
Errors which have left the Pharmacy will be documented on an incident report form and reviewed monthly to identify learning outcomes.
Once the Near Miss Improvement tool have been completed a copy is now required at Head Office where it will be reviewed and learning points can be shared with the other branches.

The pharmacy does not keep the necessary records to help manage its services safely.

Standard Operating Procedures will be reviewed
Procedures which are duplicated will be removed and archived so there is clarity on the agreed, safest and most effective procedures.
Staff will be asked to read and sign procedures after they have been reviewed.
Responsible Pharmacist will be directed to complete CPPE e-learning for Patient Group Directions to make sure he is aware of all requirements.

The pharmacy does not provide people with easy access to the information they need to help them take their medicines safely.

The Pharmacy team will Provide Patient Information leaflets to all Patients receiving Multi-Compartmental compliance packs on a Monthly Basis.
There will be a notice displayed in the dispensary to imply the importance of this.
We will display a poster in the Retail Area and communicate verbally to patients that we can Print Information Leaflets on Medicines or Medical Conditions as they require.

The pharmacy does not monitor temperatures in the fridge. So, it cannot know if the medicines stored in the fridge are safe to use.

We will purchase a new Fridge Thermometer with a Min/Max function and Staff will take readings on a daily basis. Head Office will request copies of readings on a monthly basis to ensure compliance.
