We have fallen out of the habit of writing up near miss errors however in the last year however this has been discussed in our last staff meeting and all members have been reminded of the importance. High risks medicines namely controlled drugs which are to be consumed on a daily basis are stored during the day in the CD cabinet but are taken out and placed in the back dispensary until we obtain a new CD cabinet for the front dispensary. We have made necessary arrangements to make sure that these medicines are out of the patients view. |

We fell behind writing up records half way through the Covid Pandemic. We have created a new system which entails a new end of day procedures which would act as a reminder to make sure that all records are kept up to date. |

We have learned how to create batch labels on our new PMR system which label Batch numbers and Expiry dates. If we are storing any high volume medicines in containers we can now create our own batch labels to identify original containers. |
