The pharmacy has a written risk assessment in place for its prescribing service. But it does not effectively manage the inherent risk for some of the identified risks to help ensure services are provided safely. For example, some people may be able to obtain medicines which are not suitable due to inadequate checks being completed.

Weight Management service Risk Assessment:
*Inaccurate BMI leading to inappropriate supply will be managed by a mandatory independent verification of height and weight (see below)
*False declarations of height and weight by patients will be controlled by use of real time photos (see below)
*Verifying information provided:
- GP record checked to verify information provided on consultation form by patient
- Notification of treatment letter sent to patients GP surgery directly when treatment is approved. GP asked to respond if they feel there is a reason why the patient should not be on the weight loss treatment.
* A 2 way consultation via an online video will be carried out with new customers, enabling verification of BMI in real time.

The pharmacy undertakes some independent verification checks for some people who use the pharmacy’s weight loss service. But it does not have an effective method to independently verify the height and weight for everyone who uses the pharmacy’s services. So the pharmacy cannot provide assurance it only supplies medicines to people who meet the service criteria.

*Verification of height and weight will require one of the following,
- GP record checked, if available, to verify information provided on consultation form by patient.
- Live verification of height and weight with clinicians, via a face to face video consultation where patients BMI can be verified
-Real time photographic evidence of full body image with scale reading plus video upload of patient weighing themselves. This eliminates and reduces the risk of the video upload being AI generated / tampered with, or an older video being used. The real time photograph supports the weight video in real time.
- Real time photographic evidence of full body image verifying height and tape measure.
-Height verified by a pharmacy / GP practice, signed, stamped, and dated by the professional.
