A 'Further Consultation Notes' section has been added to the Weight Management Service questionnaire. This will allow details of discussions and advice about lifestyle to be recorded. If the patient provides consent for the pharmacy to contact their GP this will also be recorded and a GP notification slip will be completed. |

A controlled drugs balance check will be carried out every 6-8 weeks and recorded in the CD register. The nature of the emergency will be recorded for all emergency supplies going forward. |
