The pharmacy's records are not always adequately maintained. A review of controlled drug (CD) registers identified some inconsistencies including frequent missing entries and amendments without explanation. This means the pharmacy may not always be able to account for CDs and it cannot consistently demonstrate safe management of these medicines. In addition, the incorrect prescriber is sometimes recorded on private prescriptions records which could make it difficult to clarify matters in the event of a query.

Pharmacy staff have reviewed SOP in relation to entry of CD register and requirements.


A notice of common controlled drugs that need entering by the pharmacist are on display. This will ensure no CD entries are missed or not handed to the Responsible Pharmacist for entry.


A Monthly reconciliation programme between balance and physical stock has been implemented. This will be undertaken by the manager or regular pharmacist.


All CD entries are to be done by responsible pharmacists before shift ends. A reminder is placed on the pharmacist desk with CD books
relocated directly behind the pharmacist to encourage entries.


All staff have read and signed SOP relating to entries of private prescriptions. A check list for Private scripts detailing essential information that needs to be entered is available on the main labelling computer.
