The pharmacy does not adequately identify and manage the risks associated with its services, particularly its face-to-face and online prescribing services. It does not sufficiently monitor the safety and quality of its prescribing service. And the notes it keeps for this service do not always contain the relevant information. Taken together, these increase the risks to people using this service.

The pharmacy has reviewed the risks associated with its services. And as a result, Burwash pharmacy now only offers a face-to-face consultation service in order to reduce risks associated with its’s prescribing service. The pharmacy no longer supplies medicines online and the website has now been closed.

The prescriber has reviewed the NICE guidelines for UTI, STI and migraine. And has made notes for diagnosis, assessment and treatment of these conditions. Following this learning, the prescriber has now decided to limit their scope to travel medicine as from September 2023.

The pharmacy cannot sufficiently demonstrate that it monitors the safety and quality of its prescribing service, for example by undertaking and documenting regular clinical audits.

The pharmacy undertook a clinical audit in October 2023. It was not carried out earlier as there were very few patients being prescribed medicines at Burwash Pharmacy - a total of eight patients since May 2023.

Clinical audits to be repeated every 6 months or after every 20 patients, which ever is sooner.

For clarity as only face-to-face consultations are done at Burwash Pharmacy, consent is sought when patient is seen.

Please note that for almost all patients seen at Burwash Pharmacy, the diagnosis had already been made by their GP and prescriptions were issued in circumstances where an emergency supply may have been suitable.

The pharmacy's consultation notes for its prescribing service do not always contain the relevant information.

The prescriber has reviewed the Prescribing Competency Framework for prescribers. And has reviewed what good prescribing notes entails.

The consultation notes template has been amended to improve the quality and safety of prescribing and to ensure that the relevant information is recorded. And the notes will be amended to state that if the presenting complaint is a previous diagnosis or a new diagnosis. And if the medication is a repeat medicine or a new medicine. Differential diagnoses will be included in the consultation notes.

The prescriber has arranged four sessions with their designated medical practitioner where they spend the morning in clinic and observe in particular the note taking for the various patients seen.
