The pharmacy cannot clearly demonstrate how it manages the risks associated with its pharmacist prescribing service. It does not have any procedures explaining how this service operates and there is no evidence that it has been properly risk assessed.

We are following the repeat prescribing procedure that is set out by our associated CQC registered clinic.
A risk assessment audit will be carried out regularly/ monthly the results will always be shared with the team to make sure risk is always kept to the minimum.
The prescribing pharmacists are using history taking/ consultation records. The patients will also sign consent for the repeat medications issued which is clearly stated as a repeat prescribing. We always ask to see evidence from to support the supply of repeat medication.

Some of the pharmacy's private prescription and emergency supply records are inaccurate or have missing details. The pharmacist prescribing service does not keep clear records to justify or document prescribing decisions.

The phone consultation will be recorded as an emergency supply based on the doctors request, which will be redeemed form the system once the Dr sign the scrip within the agreed time scale.
The private prescriptions will have the relevant name of the independent prescriber as the prescriber. Regarding the patient address, we will be using the hotel address/ room number in addition to their country of origin.
Consultation records will also be stored electronically using encrypted file. In the past paper format consultation records have been used.
