Medicines returned from care home are not dealt with appropriately. The pharmacy does not always store CD medicines securely and in line with safe custody regulations. The pharmacy cannot provide assurance that the temperature of the medical fridge is appropriately monitored.

All CDs are stored correctly in an authorised CD Cabinet and all CDs that were returned from care homes have been safely and securely destroyed and entered into the CD destruction book. Following the inspection visit new policies and procedures have been put in place. All staff have re-read the CD returns SOPs and team briefs have been held to ensure that no such occurrence happens again. All care homes have been contacted and advised to segregate and return CDs separately. The RP, the pharmacy director (PT) and manager have all been spoken to and told that any medicine being returned has to be checked by the RP who has the overall responsibility and then taken upstairs for disposal. Any future occurrence will result in serious displinary measures as any sort of diversion is possible and patient care would be compromised.

The Fridge temperatures are recorded on a daily basis and logged, upon the visit from the inspector staff were unable to locate the log form. It is now kept on top of the fridge. All temperatures are within 2-8 degrees.
