The pharmacy doesn't fully manage all the risks involving sales of codeine linctus. And it doesn't have ongoing checks to make sure the changes it makes fully reduce the risks to people taking a medicine that may cause them harm. The pharmacy doesn't provide pharmacists with information in its written procedures on how to suitably manage requests for codeine linctus.

• Incorporate a specific section in the SOP’s describing the process of any future sales of Codeine Linctus with the presence of the acting RP.
• A new risk review will be undertaken on the risks associated with the sale of codeine linctus, risks of selling codeine linctus without following the SOP and any risks regarding codeine linctus and its abuse potential. (See attached document for further details on the Risk Review Assessment).
• GPhC Zoom Meeting– A live broadcast video titled ‘Sales of Codeine Linctus - is it a cause for Concern?’ that can be viewed by all the staff in the Pharmacy, which will give them a better understanding and an insight of the effects and misuse of Codeine and Promethazine. We will enrol on this event on the 9th December
• All orders of any Codeine Linctus that could previously been done through Proscript will be locked and only overridden by the pharmacist present.
• All wholesalers will be alerted and a cap of 5 a month will be initiated and any orders place above that will warrant a call to the SI by the wholesaler.

The pharmacy does not have the necessary ongoing safeguards in place to adequately manage the safe supply of codeine linctus which is a medicine liable to misuse.

• All staff will be trained extensively of any Codeine Linctus sales, and will be given relevant information in regards to Inspire – a Prevention and Recovery facility where patients can be signposted if they’ve visited the pharmacy numerous times and have been refused.
