Hexham Pharmacy (1099926) - Improvement action plan

The pharmacy does not maintain appropriate records when supplying people with prescription-only medicines via a patient group direction (PGD). Records do not contain clear detailed history, treatment goals, ongoing monitoring requirements or consideration for any follow-up required. This means important information is not available to ensure the pharmacy supplies its medicines safely.

The Superintendent pharmacist will produce a SOP for the Pharmacy, all team members will be able to read and continue to signpost interested patients to the Responsible Pharmacist.

Both initial and follow up consultations to be recorded in patient’s notes in the weight management file. Treatment goals, progress update and questions and answers to be noted on paper, any follow up queries to be noted. Also continue gaining consent to notify patient’s GP of the PGD supply via email or letter.
