The pharmacy does not maintain accurate and complete records in accordance with regulatory requirements. The Responsible Pharmacist (RP) record contains omissions and inaccuracies, and the RP’s name recorded as being on duty is not always correct. Additionally, some private prescription records are incomplete or inaccurate. These issues make it harder to rely upon the records and could cause confusion about who was responsible or make it harder for team members to respond to any queries or concerns.

•The superintendent’s team will work with the locum coordinators to make sure the responsible pharmacist covering the regular pharmacy manager uses their own login. This login is currently tied to the RP record.
•The pharmacy manager has removed the password autosave function from the web browser, to prevent locums using the incorrect login.
•We will also audit the RP log on these days to make sure the correct login has been used, and the record is correct.
•We have asked the PMR provider (Apotec) if they can improve the RP log in process for the responsible pharmacist.
•The team will make sure that the correct prescriber is entered when dispensing private prescriptions.
•The pharmacy manager has held a team meeting about the importance of updating the prescriber so that the electronic register entry is accurate.
•The pharmacy manager will monitor the team’s compliance with this aspect of dispensing private prescriptions.
