The pharmacy does not keep all legally required records, including for some higher risk medicines. And some of these records have not been made for a significant period of time. So, as well as not meeting its legal obligations the pharmacy does not have a robust audit trail on such medicines which may impact on patient safety.

The pharmacy has started a new CD register for Methadone. The Accountable Officer has been notified of the matter and has initiated further investigation. All requested contact details and additional information have been provided to support this process. The incident has been reported via the NHS controlled drug reporting portal. Going forward, the Responsible Pharmacist will ensure that all methadone received and supplied is recorded on a daily basis. In addition, a running balance check has been implemented to maintain accurate stock control. Posters will be displayed within the pharmacy to emphasise the importance of accurate record keeping in the methadone register, and to clearly identify to the Responsible Pharmacists, particularly in situations where locum pharmacists are on duty.
