1.1

The pharmacy doesn’t identify and manage key risks with its services. The team doesn’t always work in the safest and most effective way. The pharmacy team are working under pressure to dispense a backlog of prescriptions. And so other tasks are not being completed fully or regularly.

• Backlog to be cleared utilising Relief Dispenser and double Pharmacist cover
• Experienced Rowlands Pharmacist Manager to work alongside the team and support the new branch manager to reinstate core company processes
• Make use of available technology through our off-site dispensing solution (MediPAC) to reduce the number of items dispensed locally. This will provide time for the team to complete other essential activities

There is no evidence that pharmacy team members complete or share learning when they make mistakes. And, there is no evidence of any changes being made to improve and make pharmacy services safer.

• The two errors not recorded or investigated from October and November have now been reported, with key learnings and actions identified
• All seven errors from September to be reviewed and key learnings shared with the team
• Near miss process to be reinstated with colleagues recording their own mistakes and providing sufficient detail to enable team learnings
• Monthly Patient Safety Reviews to be completed for November and December and each month going forward

The pharmacy is not utilising the skills of its team to ensure the safe and effective provision of its services. Due to a backlog of work the available team members work under increased pressure and cannot complete the work in a timely manner. And they don’t work in the most efficient way. The team are not completing other required tasks regularly or completely.

• Backlog to be cleared utilising Relief Dispenser and double Pharmacist cover
• Experienced Rowlands Pharmacist Manager to work alongside the team and support the new branch manager to reinstate core company processes
• Implement a task rota to help the team ensure that they are completing other required tasks regularly and completely

The pharmacy has significant amounts of clutter on benches and floors in the area where prescriptions are prepared. And this means there is a significant risk of pharmacy team members tripping and falling over dispensing baskets on the floor. And, there is an increased risk of mistakes being made with the dispensing of people's prescriptions.

• Make use of available technology through our off-site dispensing solution (MediPAC) to reduce the number of items dispensed locally
• Complete a de-clutter of the pharmacy premises to remove redundant materials and paperwork
• New task rota to include specific cleaning activities to be completed by colleagues within the branch. This will be monitored by both the branch manager and the Regional Leader

The pharmacy does not adequately manage its stock ordering. And, this means the drawers where it stores its medicines are cluttered and overfull. There is an increased risk of mistakes being made whilst dispensing.

• Team to have a briefing regarding the importance of tight stock control and all orders are to be reviewed prior to transmitting to see if replacement stock is required
• All medicine storage areas are to be reviewed and re-organised where appropriate to help to identify and manage the risks, for example, drawers will be re-arranged to ensure stock is clearly organised to support safe dispensing
