Not all pharmacy team members are trained in accordance with GPhC minimum training requirements and have not read the pharmacy’s written procedures relevant to their role. So they may not have the required knowledge to carry out their role safely in delivering services.

The pharmacy driver has been enrolled on to an accredited training course for the pharmacy delivery drivers.

The pharmacy does not have a complete set of current written procedures, including for responsible pharmacist regulations and controlled drug management. And the team do not always follow previous versions of these procedures including completing balance checks for the management of higher risk medicines. This means that the pharmacy is not always working in a way to suitably manage risks associated with its services.

The SOPs for Responsible Pharmacist and Dispensing Controlled Drugs have been printed and located in the SOP folder. All staff to read and sign these SOPs.

The pharmacy does not keep accurate records for some of its higher risk medicines requiring safe storage. And it does not make regular checks of these records against the physical stock held. As a result there are discrepancies in the pharmacy's records and no assurance these medicines are managed safely.

A full CD balance check carried out. Any discrepancies have been fully resolved and the CDAO has been contacted and has been informed of the discrepancies and the resolution(s). All team members briefed on process to follow that the balance for a particular CD should be checked prior to handing out the corresponding prescription to a patient. A full CD balance check to be carried out once every month.
