The pharmacy does not manage risks as it does not follow standard operating procedures for all activities. It also has unmanaged risk due to poor record keeping.

The standing operating procedures as updated ,will be sanctioned and adhered to by all members of staff, for all activities. The record keeping particularly for serial dispensing will be constantly and accurately updated

The pharmacy does not monitor and review dispensing accuracy. So team members cannot learn from mistakes and improve the service.

The Pharmacy constantly monitors and reviews dispensing accuracy and the entire team is notified at the time of the error and possible ways to learn from the mistakes. We will now also have a more formal meeting from time to time to discuss possible improvements.

The pharmacy does not keep accurate records. This leads to confusion and increases risk that people may not receive medicines safely.

The pharmacy does not always supply medicines safely as noted elsewhere. It does not check that items it stores are stored at the correct temperature.

We have invested in a pharmacy refrigerator which displays the temperature constantly and also has an alarm if the temperature varies beyond the required scale. There is also a USB monitor for max and min temp.This device needs an instruction manual. In the meantime manual records will be kept
