2. Provide Pharmacy Store Manager with the identified skill mix and hours for the pharmacy.
3. Regional Development Manager to maintain contact with the pharmacy on a 4 weekly basis ‘How you doing – check in’

The pharmacy does not routinely review the reasons for dispensing mistakes so cannot learn from these.

1. Organise team meeting, discuss Patient Safety reporting requirements, near miss recording & reviewing.
2. Complete reflection and learning – weekly share & learn team huddles.

The pharmacy does not have enough suitably qualified staff to safely lead and deliver its services.

1. Review staffing skill mix and hours for the pharmacy.
2. Review region escalation process and provide updated version to Relief Managers and stores.
3. Complete the company staffing and capability review, agree an action plan and implement ongoing staff rotas - six weekly planning.

The pharmacy does not deliver all services safely, especially medicines supplied in instalments. And it does not follow a robust process for clinically assessing prescriptions.

1. Complete reflection and learning, agree any training required and discuss organisation of workload.
2. Re-read SOP 16, Appendix. Implementing use of PC 70 form for instalment prescriptions.
3. Re-read SOP 3 – Prescription Labelling, making the pharmacist aware of any new items, changes to medication, interactions, warnings and messages.
