1.1	The pharmacy does not keep its policies and procedures up to date, and there is evidence that team members do not follow them.	All the SOPs were reviewed/updated and re-signed by all staff members We also created missing SOPs that were not there during inspection (e.g. SOP for blister packs)	08/11/2023	25/10/2023
1.2	The pharmacy does not routinely record near miss errors and dispensing mistakes. And there are no arrangements in place to learn from things that go wrong.	We printed off Near Misses log book. All the staff were informed of the log book, and inform the locums about it. Plan is to do a report quarterly to identify most frequent near misses.	08/11/2023	25/10/2023
