Following the inspection, we realised the correct way to enter responsible pharmacist (RP) on our patient medication record (PMR) system was by logging in and logging out every day. However, we have now resorted to a paper format for this log. This will ensure we do not forget RP entries allowing them to be accurate and completed in full. We will make sure our controlled drug (CD) register will have entries made in full, which are accurate, complete and reviewed. We will carry out regular balance checks to ensure correct levels of stock are reflected. Private prescriptions entries will also be completed in full, with accurate details on the PMR. Specials records will also be completed in full, ensuring the source, date received, date dispensed, patient details, quantity, batch number and any suspected adverse reaction to the product supplied are entered. Emergency supply records will also be reviewed, making sure entries are accurate and complete knowing whether the supply was requested by the patient or prescriber. |
