The pharmacy does not store its confidential waste or other confidential information appropriately.

Confidential waste to be moved to the store room, and staff informed to ensure that it clearly marked as “confidential” and kept separate from normal waste.


All locums are required to bring their own smart cards. Whilst there are circumstances where the forge or leave the card at previous locum engagement. They should not have access to the spine. SCR requires patient approval and signed agreement to access these records. Only RP to access unless locum authorised.

Up-to-date standard operating procedures (SOPs) are not readily available and have not been read by all team members.

New set of SOP’s are being prepared incorporating national standard. These will be revised to amend for our practice where appropriate, will be read and signed by all team members. There is an updated section on CDs, to include balance checking. CD balances need to be checked by both RPs

Both teams will ensure complete information about private prescriber

Although RP notice was on an older notice, this has been replaced by a new format.

Staff to minimise distractions to reduce near misses etc. Previously mental breaks and look alike, sound alike were discussed.

The pharmacy stores medicines in areas of the premises where there is risk of unauthorised access.

Open totes containing prescription to be removed from consultation room and to clear dispensary and storeroom to make space for this.

The pharmacy cannot demonstrate that it takes the appropriate action when it is suspected that medicines or medical devices are not fit for purpose.

We are no longer providing the blood pressure monitoring service, and therefore the BP monitor is no longer required, and it will be disposed of properly.

Actioned Pharmacy safety alerts and recalls to be printed off and stored in file folder, to confirm that they have been actioned

Patients on long term would be counselled where appropriate e.g. long term Simvastatin prescribed Clarithromycin or more recently Colchicine prescribed for familial Mediterranean fever.
