Forest Dispensing Chemists (1040208) - Improvement action plan

Standard not met Reason Action being taken by the Pharmacy By when Notification By Pharmacy Improvements Made
1.1

The pharmacy cannot sufficiently demonstrate that it appropriately identifies and manages the risks associated with its prescribing service. For example, it has not undertaken a risk assessment for the service and does not have a prescribing procedure. And it does not routinely share information with people’s regular prescribers about medicines that have been prescribed.

To address issues to ensure safety, compliance, with a high standard of patient care.
To identify risks associated with prescribing service, such as potential safety and operational risks whilst carrying out the service competently and ensuring that the standards currently not met are met at high standard.
In order to do this, I will be implementing the following to my practice.
A standard operating procedure, a service risk assessment, record keeping guidance, medical history consultation checklist, prescribing incident protocol, prescribing service checklist, GP engagement letter, GP summary template and a business continuity plan.
Upon prescribing outside my scope of practice I will identify my own learning needs, by undertaking the relevant training to ensure competency as I plan to prescribe.

1.6

The pharmacy does not keep all the appropriate records necessary to demonstrate that its prescribing services are provided safely and effectively.

As identified in 1.1, I will ensure that appropriate assurance and governance of prescribing activities and professional indemnity requirements are met at all times.
That prescribing will be only undertaken by Pharmacist Independent prescriber (PIP) and in the knowledge of the superintendent pharmacist, which would be myself.
That the PIP will prescribe within their scope of practice. To ensure their insurance policy is endorsed to cover independent prescribing.
Thorough risk assessment of the prescribing service completed as mentioned in 1.1, and regular audits of PIP prescribing activity.
Clear records of all consultations related to prescribing in the premises are documented and stored in line with record keeping requirements.
PIPs to make every effort to gain consent from a patient to access their medical records and share their outcome of the consultation with their regular prescriber.
To ensure that all prescribers are registered within the UK with an appropriate regulatory body in the UK.
