The pharmacy has made improvements to some of its records. But although its prescribing service appears to be low volume, its consultation notes for this service do not always have the necessary information recorded. For example, not all the records seen contained the reasons for prescribing or detailed the symptoms that people had. In addition, it makes records which indicate that private prescriptions have been dispensed where it is not clear if there were associated prescriptions dispensed. So, it cannot show that its records are always accurate. And it does not always make the required records about private prescriptions it dispenses in a timely way.

Since the first inspector’s visit back in October 2024, the pharmacy has since put in place a comprehensive on-line consultation software which details and captures all the needed information required for consultation.

A;so as part of the consultation mote, we always mentioned the reason ehy patient has consulted our service which is always indicated the history of complaining symptoms which can clearly be seen in our consultation notes previously provided.

But as required by the inspector, in going forward a more detailed and comprehensive symptoms analysis will be taken as part of the consolation notes.

Also on the part of records made as medication dispensed but was not the case, was clearly a mistake on our part as staff did a price check on those item put forgot the delete the records afterwards as those items were not physically dispensed by just price checked. We have since educated all staff members not to keep price checked items on the system as dispensed so that all our records in going forward will only reflect current and accurate records.

Lastly in going forward all private prescriptions will be entered on the day of dispensing as when left for the next day sometimes tends to bring about oversigts and forgetfulness.
All staff member have since been given a briefing on these issues and we can re-assure the inspector that all the points raised will be ironed out appropriately to ensure a robust prescribing service are offered.
