1.2	The pharmacy has written procedures for managing dispensing incidents. But the pharmacy team does not follow the procedures. The team does not keep records when things go wrong. And there are no arrangements for the pharmacy team members to report and learn from their own errors. So, they do not have the information to identify patterns and help reduce similar mistakes in the future.	Gather the team together and discuss the report together with its implications. Revise the SOP and update. Include that, where possible, the member of staff responsible has an opportunity to learn from their error. Have at hand, copies of the error report forms, to enable recording of mistakes as they occur.	15/08/2019	16/10/2019
3.2	The room in the pharmacy used for people to have confidential conversations with the pharmacy team does not protect people's privacy.	We intend to instruct a local contractor to apply an opaque film to the plain glass in the consultation room to gain privacy for the room occupants.	15/08/2019	16/10/2019
