Pharmacy team members have not followed procedures to remove short-dated or expired medicines from the shelves, even after receiving feedback. And the pharmacy has expired medicines on its shelves. So, there is a risk that people may receive medicines that are not fit for purpose.

The company and superintendent pharmacist is very disappointed that standard practice was not followed. Immediate investigation and remedial action has been carried out.
• All stock has been re-checked as priority.

• An immediate investigation has taken place to understand why procedures were not followed and feedback enacted.

• An immediate review and consideration has also taken place of management and staff roles, their conduct and appropriate remedial action, re-training and re-setting of expectations for future conduct.

• Experienced staff sent from another store to improve process efficiency, reset targets/ambitions and to re-train staff and regular pharmacists.

• Monthly inspection/compliance checks undertaken by the company to ensure that procedures are being adhered to going forward and that staff are coached and deployed effectively.
