But there are multiple gaps within the Responsible Pharmacist record which means that the pharmacy may not always be able to demonstrate who was responsible for the safe and effective running of the pharmacy at a point in time. And higher-risk medicines are not always audited as regularly as they should be, so any discrepancies or diversions may not be immediately detected.
