The pharmacy does not have a robust process that is routinely followed when the pharmacy team are informed that an incident has occurred. This means that the team are not completing a thorough internal investigation and opportunities to put actions in place to prevent a similar incident occurring could be missed.

Staff / Pharmacist training on complaints & incident reporting i.e. near misses, dispensing errors, complaints and any incidents. This will include SOPs, LFPSE, PMR Incident recording, cd reporting portal.
