1.1

The pharmacy team does not identify and manage some risks to people’s safety as a result of its working practices. Not all the team members have read all the company’s written procedures and some of these are not being followed.

Our newest recruit will be given 20 minutes each day to read our SOPs.
Each staff member will record every near miss and weekly team meetings will be held by manger to discuss mistakes and how to improve on them.
Manger to identity any Sops that he feels will help improve patient safety will be re-read and discussed with team.
Manager to continue to support the learning and development of team.

Pharmacy staff do not receive appropriate training for the roles they undertake as they are not enrolled on accredited training courses within three months of starting in their role.

I have spoken with HR and both apprentices will be enrolled onto buttercup training course. Our process is that when we hire a new trainee they are automatically enrolled onto Numark training and once they have passed their probation period (6 months) they will be enrolled onto buttercups training course. We have had issues with Buttercup training and we are actively looking to take courses to another company unless Buttercup improve on this.

The pharmacy’s security arrangements are inadequate as the back door cannot be properly secured.

Routine maintenance has now been completed.

Multi-compartment compliance pack dispensing is not done in accordance with the company SOPs. This increases the likelihood of mistakes, and means packs are not always assembled correctly or supplied with the right information.

Dossette boxes have now been moved back into branch and are made in the Pharmacy. The Superintendent has asked all staff to re-read Jhoots SOP on dispensing and supplying dossette boxes.
