| 4.3 | The pharmacy team do not consistently ensure that medicines are removed from dispensing stock when they are expired. Date-checking records are not available to demonstrate that stock is routinely monitored, storage arrangements are not sufficiently organised, and some out-of-date medicines were found on the shelves. This increases the risk of expired medicines being selected and supplied to patients and means the pharmacy cannot demonstrate that medicines are consistently managed and stored in a safe and effective manner. |
A new date checking matrix has been established. The previous date checking matrix was not being completed as intended. Stock is now date checked every 3 months and is split into sections. Any medicines expiring within 6 months have a red sticker attached and a log of the item made. When date checking is repeated those items are located first if still present. Date checking completed on entire branch stock completed on 7th October. This will be repeated so that it is again completed in 3 months time. |
