O'Flinn Pharmacy (1036125) - Improvement action plan

The pharmacy does not routinely record any dispensing mistakes, including ones which are found before the medicine has been handed to a person. There is some evidence that dispensing errors have occurred and the pharmacy is unable to demonstrate that records had been made about them. This makes it harder for the pharmacy to show what it has done in response to dispensing mistakes. And not recording them means it is harder for the pharmacy to identify any patterns or trends.

Ensured that a near miss log book is in place for the pharmacist and other staff to record incidents

Created an account for Learn From Patient Safety Events (LFPSE) and making sure all staff understand how to use the system and how to complete the monthly patient safety report.
