While some of the legally required records such as the responsible pharmacist log are accurate, some of the pharmacy records for medicines that require secure storage are not correct. So the pharmacy cannot provide adequate assurances that these medicines are always being managed safely.

All staff have been re-trained on correct CD handling and accurate use of the electronic CD register.
Incident reporting procedures have been reinforced to ensure immediate escalation of any future discrepancies.
Stock reconciliation processes have been standardised to align with CD regulations.
