The pharmacy doesn't have written standard operating procedures (SOPs) that reflect all the activities it carries out. The SOPs it does have, haven't been reviewed for many years and haven't been signed and reviewed by the current superintendent pharmacist. And members of the pharmacy team don't always follow the SOPs they have signed. This means the pharmacy can't demonstrate that its team members always work safely and effectively.

We have now internally agreed on downloading/making necessary changes to the most up-to-date SOPs from the NPA website and would make sure all staff working in the pharmacy read and understand the signed SOPs as instructed by the superintendent (SI) pharmacist.

We have already started signing off the ones needed for dispensing prescription items safe practice, patient safety and confidentiality, handing out dispensed medicines, legal and clinical check.

The pharmacy can't show what action it has taken about previous safety alerts and recalls. This means it can't demonstrate that it responds appropriately to safety alerts and recalls to ensure people only receive medicines which are suitable for use.

Effective immediately, the RP responsible pharmacist will download any alerts received from http://www.gov.uk\drug-dafety-update and will download, print and sign the notification (alerts, recalls and safety information: medicines and medical devices) as evidence of acknowledgement. The RP is now subscribed to the MHRA drug safety update (dsmail@mhradrugsafety.org.uk).

The SI has also now encouraged the RP to go back and check if any medicines in the pharmacy have been impacted by any previous recalls or safety updates and alert the SI for further action within 15 days beginning the 28th of Feb 2025.
