The pharmacy does not keep all its record in the way the law requires. And many of its records are incomplete. This means that it may not be able to account for all its activities day to day. This includes activities around the medicines it receives and supplies. This means that if something went wrong it would be difficult for the team to establish what happened and why. And it would be difficult to rectify it. This also means that it would be difficult for the pharmacy to demonstrate that it monitors its activities safely and appropriately.

Fridge temperatures (lowest, highest and current) will be recorded daily on our PMR system, with clear steps to follow if a reading is outside the safe range. The RP log will be brought up to date. Daily sign-in and sign-out will be done every shift, with a reminder displayed in the dispensary. A new written procedure on record keeping will be put in place and signed by every member of staff.

The pharmacy des not do enough to ensure that all its medicines are managed safely and securely. And in the way the law requires. This means that it may not be able to account for all the medicines it should and keep them safe.

A full stock check of all controlled drugs against the CD register will be carried out. Any differences will be looked into and fixed. Existing entries will be checked for clarity, completeness and legal compliance. Every two weeks, Schedule 2 CD running balances will be checked, signed and dated. A new CD procedure will be put in place and signed by all staff who handle them. The CD cabinet will be securely fixed to the wall.
