The pharmacy cannot show that it manages all the risks linked to the prescribing service appropriately. Prescribing decisions are not adequately recorded.

The pharmacy has implemented a record keeping system to enable the pharmacist prescriber to safely record consultations in line with RCGP guidelines. This includes a thorough history taking section, examination section, undifferentiated and differentiated diagnosis section, safety netting and planned section.
The record keeping system is NHS and GDPR approved.

The pharmacy cannot show that its prescribing service always protects people’s health and well-being. It does not always take all reasonable steps to confirm that the information provided by people is reliable.

The pharmacy has implemented a robust standard operating procedure for the prescribing service and a thorough record keeping process.
It obtains relevant information to aid in the decision-making process. The pharmacy will utilise reputable resources such as summary care records, with the consent of patients, in order to make correct decisions to allow safe prescribing. The patient will be given a letter to sign to enable the pharmacy to access their summary care records. Also detailed in this letter is the permission for the pharmacy to communicate with their doctor about their health.
To complete the consultation, a summary letter will be generated and will be sent to the patient’s GP to help promote continuity of care by keeping the doctor informed of care received from the pharmacy.
