We are keeping on top of our CD records. The pharmacist has designated one member of staff to help keep the records maintained.

The pharmacy provides services which involve a degree of risk, but they have not completed adequate risk assessments or given enough thought to how these risks could be better managed.

We are making sure our CD records are up to date and I have a dispenser helping and ensuring the workload gets easier by sharing these duties.

The pharmacy is very cluttered with a lot of litter kept in the yard and in the stock room. This is a hazard and could present a significant safety risk.

We have hired someone to come and clear the litter in the back yard to make it safer for everybody. We have now hired a cleaner twice a week to keep the inside of the pharmacy and the back clean and tidy.

Pharmacy services, controlled drugs in particular, are not all provided safely. There are inadequate, and in some cases, no audit trails or controls in place to verify whether or not the service has been provided, and who by.

We are looking at going through the NPA to put a service in place where it makes it easier for medications to be delivered with patient details made confidential. We have trained a member of staff to help the pharmacist to document methadone prescriptions.

There is no evidence to show date checking of medicines is carried out and some medicines are stored without the legally required information.
