The pharmacy does not have adequately robust processes for managing and storing its medicines, including checking expiry dates. There is evidence of out-of-date medicines on the shelves. It does not keep all its medicines in the original packs. And it does not properly monitor medicines stored in the fridge. This increases the risk of errors and the risk of supplying medicines to people that are not fit to use.

Company has date checking matrix in use across all branches. This branch was acquired recently and unfortunately, date checking matrix procedure has failed to be implemented. This will be rectified immediately. In reference to medication not being kept in original packs, this occurs in the event that Medipacks changes are requested from prescriber after the assembly process has been completed. This results in the medication being removed and stored in bottles. Company allows this process however stipulates that the following details are recorded on the container:
Name, Strength, Form and quantity of medication
Date on which medication was transferred
Batch Number of medication
Expiry Date of medication.
Staff will review SOPs to confirm their understanding and adhere to the process going forward.
Fridge temperature logs are maintained on the PMR system and system is set up to prompt readings to be recorded. Staff should be aware of the usage of Min-Max. thermometers and resetting following recording. This will be reviewed with all staff.
