The pharmacy does not always keep accurate records as it is required to do by law. This includes for controlled drugs and for specials medicines. This is similar to issues highlighted in the last inspection.

Paperwork has been updated and organised in such a way as to make it clear and appropriate information completed

The pharmacy doesn't always identify and manage key risks with its services. This includes the accuracy of its standard operating procedures and how the team is working. There is ongoing poor record keeping. The pharmacy does not adequately manage the risk with storing medicines outside their original containers and with the build-up of patient returns and confidential waste.

Record keeping has been reviewed, SOPs and near miss reporting is now done through Pharmapod, taking the whole system online, with clear audit and tracking capabilities. Regular staff have read through revised SOPs and signed off on them. New starter and staff member who has been of sick, both of whom started/returned to work on 13th Dec are working towards completing them

The pharmacy does not always provide a safe and suitable professional healthcare environment for pharmacy services. Some areas of the premises are cluttered and untidy. This results in the team working in a disorganised and chaotic way. And at risk of trips and falls.

Pharmacy has been tidied with staff delegated to keep on top of specific areas to ensure it remains so

The pharmacy does not always adequately store all its medicines with the proper safeguards in place. It routinely transfers medicines from manufacturer's original packaging and on most occasions, it does not have adequate labelling. The pharmacy does not adequately seperate and dispose of obsolete medicines and patient returned medicines in a timely manner.

Any medication found not to be in its original container, has been removed and disposed of
