The pharmacy does not identify and manage all the risks associated with its supplies of specific higher-risk medicines. It does not have risk assessments for the service. And it does not have relevant information, such as prescribing policies to help the team manage the risks when supplying medicines in this specialised area.

As superintendent pharmacist, I have contacted both Rokshaw and Sapphire Clinic in order to enquire whether either had a pre-existing SOP that I could possibly incorporate into our pharmacy business for providing this service. As neither could help in this matter, I have contacted the operational director of our pharmacy group for his assistance in formulating an SOP that I can follow for future provision of the service.
I have also re-instigated the matrices for CD Cupboard Checking, Date Checking and Cleaning Rota as advised by inspector on the visit.
